You have downloaded the InXite® mobile app now what?  Not to worry, we are going to help you make your way through the process.  It is really easy to create your own healthcare profile and we are going to help you step by step.  The first thing is to make sure you have your smartphone with the app downloaded and ready for use.  We have made the process as trouble free as we can.

The goal is to give you the most comprehensive healthcare record as possible when you are finished entering your information. Your data is totally safe and secure once it is in your healthcare record. The InXite® primary focus is on you, the patient.  The InXite® data platform allows you to decide who has access to your healthcare records.

The patient is empowered to make decisions concerning their healthcare journey. Get ready to enter into a world that makes sophisticated technology easy.  The journey you are about to take will give you the information you and your providers need to deliver top notch healthcare created just for you.  Let’s begin with step one creating your account.

Step 1. Create the Account

Hello and welcome to the InXite® mobile app.  You have downloaded the application and it is time to create your individual account.  The first thing to do is establish a user name and password, once you have done that you will hit submit.  Often your healthcare provider will give you a code to input. You will now see a screen that says “I have a code.” If you are creating an account without the referral of a physician, you will see a screen that says “I do not have a code.” Don’t worry if you do not have a code, just click the box that pertains to you.

After you click the appropriate box you will be taken to a screen that allows you to add basic information about yourself.  For example, here is where you give your name, address, etc.  This is the first step in establishing an individual healthcare profile.  This data is exclusively about you and begins the task of setting up a complete healthcare profile.

Your data remains safe and secure with InXite® all you have to do is give us the required information and you are on your way to a healthcare journey that will empower you to control your data.

Step 2. Creating The Emergency Health Card

The next step is creating an Emergency Health Card. This card is a vital resource for you. It allows you to have one document that contains crucial medical information such as name, address, emergency contact, primary care physician, any allergies you may have, and current prescription information.  We can never predict when we may be faced with a medical emergency that renders us unresponsive.

Once you have entered all the information into the Emergency Health Card, the card may be downloaded to your cell phone making it available to the emergency medical personnel who treat you.  If you are unable to speak for yourself this card can be a lifesaver. It also saves time when seconds are of the essence.

The Emergency Health Card is a unique feature of the InXite® data platform.  The card allows patients to have vital medical information stored and easily available through their smartphone.

Step 2A. Adding Personal Self History

Here is a step that allows you to give us a detailed look at your medical history. The more information you can provide, the more complete your healthcare profile will be. Clicking continue will take you to the final screens where you may enter your personal self history details complete the profile section.

Continue to enter in details as they apply to you. Don’t worry if you have not entered these yet. Simply click the I DO NOT HAVE radio button and hit NEXT. Clicking continue will take you to the final screens where you may enter your personal self history details and complete the section.

The profile is now complete. Either take the app tour, go on to the HRA secation or choose to continue at a later time.

Step 3. Taking the HRA (Health Risk Assessment)

You are now ready to take our health risk assessment.  This is probably one of the most important pieces of information you will complete during the enrollment process. The health risk assessment is the place where you will answer a series of questions concerning the current state of your health.

The more honestly you answer the questions, the better it will be for healthcare professionals to create a unique healthcare plan just for you. The health risk assessment is a way for professionals to analyze your chronic care risks.  The goal is for you to answer each individual question to the best of your ability.  The more information you provide, the better the outcome will be.

When you give comprehensive health information, professionals can analyze where you stand with regard to future health risks. The assessment relies on you to answer all the questions you are able to complete.  If you find a question that does not pertain to you, then you may leave the answer blank.

Step 4. Adding Your Healthcare Providers

This screen allows you to enter all the information you know about all your healthcare providers.  Start out with your primary care physician and the screen will prompt you to give as much as information as you know about your main provider.  After you do that, repeat the steps for all the specialists you see.

By giving us contact information about the healthcare professionals you see throughout the year, it allows us to communicate with them (once you give permission) so that we can compile the most comprehensive record we can for you.

Even is you have just seen a specialist once, it is important to have the information in case there needs to be follow up concerning a medical issue that you have had in the past.  With the InXite® data platform, communication is key between the providers and patient.  Don’t worry about giving us too much information or too many providers, we will use only the information necessary to create the most complete record.

Step 5. Adding Your Allergies and Medications

Here is where you are able to create a comprehensive list of all your prescription and non-prescription medications along with all your allergies.  Remember to be as specific as possible with both your medications and allergies.  Often people forget to list their environmental and food allergies along with their allergies to medications.

It is important to list all allergies because there may be instances where allergies come into play when prescribing medications.  There may also be times when medications interact with each other so it is important to list any and all medications that you may be taking at this time.

By having a comprehensive list of all allergies and medications you will have an extra safeguard against being exposed to an allergen or medication that you may be allergic to.  This will protect you in all situations especially when seeing a new healthcare professional.

Step 6. Adding Your Problem Details

Many times, we would love the opportunity to communicate with the healthcare professionals that work to make us healthier but we don’t want to “bother them.”  This section is a place for you to communicate with your care team about any medical issue that might arise.  In the notes section you may notate any problem you may be having.

It is very important to keep a record for your care team.  These medical issues are often not serious but providers want to keep a record of any symptoms that may occur.   By notating problems here as well as sharing them with your healthcare provider, you can make sure that they are added to your permanent healthcare record.

This screen allows not only you but your healthcare provider to keep track of them. The record of your symptoms may seem a small thing to do but can sometimes be important should other medical issues arise.

Step 7. Sharing Information

The last step for you to complete is designating the individuals that you would like to have access to your healthcare information.  You may change this designation at any time. You are the only person empowered to make the decision as to who you would like to have access to your healthcare data.

The patient designates which provider may see or utilize their records.  At InXite® safety and security is very important to us and we take every precaution to ensure that your information remains secure.  The data always belongs to you and is 100% portable.

Through the mobile app, you will have access to your healthcare data 24/7 no matter where you are.  You can rest assured that your records are safe and secure with InXite®.

Step 8. Adding the Goals

We all have goals when it comes to healthcare and wellness.  Here is the place in your personal health plan where you can establish a goal and add it to your permanent record.  By keeping track of your health and wellness goals, your care team can assist you in managing and achieving your goal.

The first thing to do is enter the goal.  You will also establish a due date or time line for achieving the goal, for example lose 10 pounds.  In order to achieve the goal, you may want to link it to other things like increasing your exercise or limiting the amount of carbs you eat every day.  You may enter more than one goal at a time.  Make sure that you enter the date that your goal was completed.

Once each goal is achieved you will receive notification and the completion date will become part of your permanent record.

Step 9. Adding Insurance
